Who is responsible for your information?

Bangkok Physiotherapy Center Co., Ltd. (BPC), Thai company registration number 0105564019139, is the data controller for enquiries and appointment administration through this website. Registered address: 41, 39 Boulevard Building, Rooms 2-01 and 2-04, 2nd Floor, Soi Sukhumvit 33 (Daeng Udom), Sukhumvit Road, Khlong Tan Nuea, Watthana, Bangkok 10110, Thailand.

Information we collect

When you contact us, we collect the name, phone number, email address, preferred branch and administrative message that you provide, together with your privacy acknowledgement and the time of submission. Required fields are marked; without the contact details needed for your request, we may be unable to respond or arrange an appointment.

Our hosting and security services may process IP addresses, browser/device details, access logs and anti-spam signals. The form uses Google reCAPTCHA to protect against abuse. Our website also uses cookies and may load analytics, maps or other embedded services according to the choices and controls available on the site.

Do not send medical information through this website or email

The website contact form and ordinary email are for administrative enquiries only. We do not request or intentionally collect medical information through these channels. Do not send symptoms, diagnoses, medical history, test results, treatment photographs, prescriptions, identity documents or insurance claim files. A website enquiry is not a medical consultation.

If clinical information is necessary, our team will explain the appropriate clinical process separately. If you send medical information without being asked, we will restrict its handling, contact you about a suitable channel where needed, and remove it from ordinary enquiry records when it is no longer necessary, subject to applicable legal obligations.

Why we use your details

• To answer your enquiry and contact you about the service you requested.
• To arrange, confirm, change or cancel an appointment and send relevant appointment reminders.
• To prepare administrative receipts or invoices for an insurance claim when you ask us to do so.
• To keep the website secure, prevent spam and meet applicable legal obligations.

Under Thailand’s Personal Data Protection Act B.E. 2562 (PDPA), the relevant grounds depend on the purpose: steps you request before receiving a service or performance of our service agreement; legitimate interests in secure administration where these do not override your rights; legal obligations; and consent where required. Where we rely on consent, you may withdraw it. Withdrawal does not affect earlier lawful processing.

Submitting this form does not subscribe you to marketing. We use your contact details for the enquiry and related appointment administration, not to add you to a promotional mailing list.

Who receives information

Authorised BPC staff and the selected branch may access information needed to handle your enquiry. Hosting, email, website and anti-spam providers may process information on our behalf or under their own applicable privacy terms. Access is limited to what is needed for their function.

We share information with an insurer only at your request or with your authorisation for the insurance assistance you ask for, and only the information needed for that purpose. A form submission does not authorise disclosure of medical records to an insurer. Any clinical documentation and additional authorisation are handled separately. We may also disclose information where required by law. We do not sell your personal data.

Some service providers may process information outside Thailand. Where a transfer is subject to PDPA requirements, appropriate safeguards or another permitted legal basis must apply. Google’s Privacy Policy and Terms apply to reCAPTCHA.

How long information is kept

Our retention period for ordinary website enquiries and related administrative correspondence is 12 months after the enquiry is closed. We then delete or anonymise them unless a legal obligation or an unresolved dispute requires longer retention. Any longer retention is limited to the relevant purpose.

Clinical records, statutory accounting records and insurance documentation managed outside this enquiry process follow their separate applicable legal retention requirements. Technical security logs are retained only for the period needed for security and troubleshooting. Restricted backup copies are removed through the applicable backup cycle and are not used for routine correspondence.

Your rights under the PDPA

Subject to the conditions and exceptions in the PDPA, you may request access to and a copy of your data, correction of inaccurate data, deletion or anonymisation, restriction of processing, objection to processing, and data portability where applicable. You may withdraw consent for processing based on consent without affecting prior lawful processing.

We may ask for proportionate information to verify your identity; please do not email medical records or identity-document copies unless we have agreed an appropriate secure method. We respond within the applicable legal time limits and explain any lawful refusal or limitation. You may complain to Thailand’s Personal Data Protection Committee or its Office; information is available at pdpc.or.th.

Children, security and changes

A parent or legal representative should contact us where consent or an appointment request must be made on behalf of a child or a person unable to consent under applicable law. The form is not a substitute for the clinic’s consent procedures.

We use access controls, encrypted website connections and anti-spam protection to reduce risk. Ordinary email should not be used for sensitive information. We may update this notice when our practices or legal obligations change; the effective date appears above.
